How To Get Your FREE Midphase Affiliate ID:

First Go To Midphase and setup your Free affiliate account.

Then simply login to your midphase affiliate account and enter the numeric
numbers in the top right corner of your account screen below which looks like this...

in this example you would enter 0.353247878913866 in the space below

Please Note: Once you submit the form your website will be uploaded automatically. ANY content on your server will be overwritten. This is meant for NEW Web hosting Customers of MIdPhase Hosting and will NOT work with any other host. If you setup a Midphase hosting account through the link on our website and you get an error when you submit the following form PLEASE wait 24 hours and try again. If your second attempt does not work please use the ticket system and submit a trouble ticket with ALL of your web hosting information.
